Tulsa pain management : Ep-7 Part 6
I do a trial make sure that they tolerate the side effects which can be significant and we plan to pump fill it up with a drug and then again we adjust the dose according to patients response Okay so I called Tulsa pain management.
I’m considering having a pump pain pump put in. How does the pump get put in my body. What how does this work.
So how does it come easy or you know again just like the stimulator making decisions. Typically one in the back where you put the cath in using a needle so we put needle into the spinal canal. The fluid space and mean we don’t we don’t as you put in the spinal cord. And in this thread the plastic character in an abstract we make a pocket you know either in the belly or the side of the flank for the pump and then we connect you to get her in and again everything is under the skin there is nothing that’s exposed to the outside. And so just like the stimulator you know it’s a minor surgery we have to just come in the morning or afternoon and then they are home by the middle of the day or the end of a day getting some pain medicine to recover while the wound heals and go home the same day itself came the same question always patients come in and all of the management I had this pain place.
How long can I expect to be off work.
How much time should I request again is about the same you know between three to three to seven days. It’s just the recovery of the wound you know it hurts especially with a pump pumps a lot bigger than the stimulator so it’s obviously you know when you move your body it’s going to hurt. So we typically are sufficient again you know to be off work within three to seven days. And not to engage in any heavy activity for at least a month after the implant.
Think it’s also the Tulsa pain management I have this pain pump put in I have all this medicine put in the pump. How long does that medicine last.
So depending on the dose it can last between you know three months to a year. We don’t let a person go to a year because the medication degrades the time because the body temperatures over time you know drugs can chemicals do break down. So we have patients come in every three months to have medications switched up again depending on a dose. I typically don’t put it in high doses so I’ve not had any since when I had to have come back every month although you know with high doses obviously high dose requirement then the pump has to be refilled more frequently. But in my practice it’s between three to six months OK. So
how does the Tulsa pain management I have this pump put in. How’s the medicine put into my pump to refill it. Is it another surgery. Has
it refilled the pump at the center pump there is a silicone membrane that that you could puncture multiple times without causing leakage. So we use a special needle and the needle does not cut the core out the silicone membrane so it. So what we do is we will be implanted. The pump is not too deep and away from the skin so you can feel where the center of the pump. And then we use a needle we pierce the skin of obviously we you know we do in this condition we clean it you know and we puncture the skin and go into the membrane. And so we feel it through the skin well through the skin and into the membrane director says No sorry you had to cut. You know you don’t have to cut it open to refill the pump and lot you tolerate the pump feel really well because the needle again is very small. It’s just a small tiny little puncture. You know it doesn’t take you long to find the port. So it’s we do it in the office.
Most people told it really well OK what it tells a Tulsa pain management when we tell our patients our risk or concerns with that with a pump just like it does so because of medication they’re delivery is very very potent.
So you know these pumps have been around for many years for decades so that they have a very good record of safety. So I’ve not had this is a pump malfunctions or deliver too much medication. What you tell patients is that be careful what you take in addition to what’s in the pump because you know this is a very potent medication being delivered directly to you. And of course you take any additional medicine that has the same effect then you could you could easily overdose on that. You know the risk of infection usually happens like shortly after surgery. You know we didn’t. A couple of weeks. But once the pumps are dead as well formed the reason infection is very low. We recommend using a sterile technique when we’ve really filled a pump so the infection should be a very low risk with a pump like Tulsa pain management.
I had this pain is it inside me for the rest of my life is that it doesn’t ever need to be replaced.
The pumps lasts up to seven years or so. So we and to replace me every seven years and usually the pump is a little computer in there it tells you when to replace it. So yes the seven years is what we. How long do you expect the pump to last.
So a patient comes at all the Tulsa pain management they have a pain pump put in and we find out they’re allergic to one of the drugs on the pump. What happens at that point.
Then we switch off the drugs. The first thing to do is to make sure that the patient is allergic to the drug so we take a history make sure. Ask me how long they’ve been if had the medication and as sort of side effects it truly as you recommend you get a rash or you get in severe cases you know low blood pressure. You know you get like swelling all on the body. It is indeed truly an allergic reaction. We we ask you know what what how severe it is and it can be very mild. We have to take some anti-histamine or anything like you know. An end to each medication. And typically a lot of these mild allergic reaction to kind of go away with time for the severe reactions then we we switch out the medication to wash it out and we consider it an alternative medication.
